Support for option groups

In the case of html options this should support the `<optgroup>` tag, for other data sources consider a format something like:

  {optgroup:'Group 1 name', options: [{text: ..., value: ... }...],
  {optgroup:'Group 2 name', options: [{text: ..., value: ... }...],
]
```
