How can we reproduce it?
Unsure how to reproduce, it just happens every few hours or so.

Bump - still an issue.

Does this issue still cause problems on 4.6.2-snapshot with paper 1.16.5? (+ if you have a proxy with waterfall/velocity then use latest)

Just got it with paper-1.19.2 and ViaVersion-4.6.1
Log is like:

UUID of player <USERNAME> is <UUID>
<USERNAME> lost connection: You logged in from another location
<USERNAME> left the game
[ViaVersion] Duplicate UUID on frontend connection! (<UUID>)
<USERNAME> joined the game
<USERNAME>[/<IP:PORT>] logged in with entity id <...>

*Note: <USERNAME>, <UUID> and <IP:PORT> are placeholders and were replaced accordingly

Maybe the ViaVersion warning comes because of the login from another location?
