finish TSP table schema templates

Complete 02-tsp-create-schema-template.sql.

As most of these can be copied directly from the BSP and just adding years, I've been trying to only do these once BCP is finalised, and BCP is undergoing a transition to Census Dictionary Tables rather than simple ENUMs so isn't finalised yet.
