Track and increase test coverage #424
Description
I'm discovering either dead or untested code paths on a routine basis.
It would be extremely useful to
- implement a coverage report
- enforce no coverage degradation in CI
- review all coverage misses and remediate them by
- removing the unused code path, or
- add a unit test for it, or
- consciously label with
#pragma: nocov.
