Missing posts in Home timeline

Steps to reproduce the problem

1. Open https://mastodon.social/home

Expected behaviour

All posts from followed account @mjtsai are displayed

Actual behaviour

Only 2 of 4 recent posts from followed account @mjtsai are displayed

Detailed description

This issue also affects my native third-party client Mona, so it's not just the web interface.

I checked all my mute filters, by there's nothing that would match the missing posts.

If this is happening on your own Mastodon server, please fill out those:

• Ruby version: (from ruby --version, eg. v3.4.4)

Follow-up: I've discovered missing posts in my Notifications too. This happens on both the web and in a native app, and the posts do not include any of muted words.

Notice also in the attached screenshot that there is no "1" after my post to indicate that there is 1 reply.

And I just discovered a private mention that I wasn't notified about either. Something seems to be seriously wrong.
