Add myaccessid proxy

SRAM MyAccessID proxy

Deze Ansible code is inmiddels vanuit de branch gedeployed op test2 door het volgende te runnen:

./provision test2 --tags midproxy --limit docker_apps1 --check --diff
./provision test2 --tags midproxy --limit docker_apps1

Daardoor is de proxy te bereiken op https://midproxy.test2.surfaccess.nl/idp/proxy.xml. Hij is ook bij MyAccessID aangedragen als valide een OIDC RP, waardoor de hele flow te testen is door te gaan naar bijvoorbeeld https://profile.test2.surfconext.nl/, te zoeken op SRAM MyAccessID Proxy in de lijst van IdP's, dan bij MyAccessID weer te zoeken op SURF, en dan in te loggen.
