fix(omp): make effect driving durable and deterministic

Summary

• add the OMP deterministic effect driver with bounded shell execution and durable checkpoint recovery
• preserve exact per-effect model routing and structured agent prompts
• enforce retained single-writer agent ownership, immutable results, and automatic continuation
• ship the driver and blocking bridge agent in generated OMP packages

Superseded by the combined upstream integration PR #1582, which contains this driver commit and the integration observability commit on top of it.
