Review Summary
Great work on this solution! The code is clean, well-structured, and correctly implements almost all of the requirements.
I am requesting one small change to fully align with the task's visual example. Currently, the labels are added after the input fields because you're using the .append() method. The expected result shows the labels appearing above the inputs. To achieve this, please use a method that adds the new element to the beginning of its parent, like .prepend().
Once the label placement is updated, this will be ready for approval. Keep up the excellent work!

The .append() method adds the label as the last child of the parent element, which means it will appear after the input field in the HTML. The expected result image shows the label appearing above the input. To achieve this, consider using a method that adds the element at the beginning of the parent, such as .prepend().

Review Summary
Excellent work! I am approving your solution. You've successfully addressed the feedback from the previous review by using .prepend() to correctly position the labels before their inputs. Your code is clean, efficient, and fully meets all the task requirements.
You may notice a failing check run, but this is not an issue with your code. It's a temporary problem with the CI environment configuration, and your implementation is correct. Great job!
